CLASS A/B AND C PHYSICALS

The United States Property and Fiscal Office for Florida (USPFO-FL) is issuing this source sought notice as a means of conducting market research to identify potential sources having an interest and industry capabilities available to Perform Class A/B and C physicals for newly appointment Florida National Guard Employee’s.

The result of this market research will contribute to determining the method of procurement if a requirement materializes. Based on the responses to this sources sought notice/market research, this requirement may be set-aside for small businesses (in full or in part) or procured through full and open competition. All small business set-aside categories will be considered. Telephone inquiries will not be accepted or acknowledged, and no feedback or evaluations will be provided to companies regarding submissions.

BACKGROUND

The Florida National Guard has a requirement to perform new hire physical’s at least twice (2) a month on Tuesdays (preferred). This will work in accordance with the pre-employment orientation with HRO. There is one type of physical that would be required, 1: Type A/B physical which requires a spirometry, audiogram and OSHA paperwork completed to include an OSHA Respirator Questionnaire. Collection of the completed paperwork would be coordinated at the end of each month along with verification of invoice and payment for the preceding month.

REQUIRED CAPABILITIES

Be able to perform this service with-in a 15-mile radius of St Augustine, 32084. Perform Medical review, Physical exam, Spirometry, Audiogram, and OHSA paperwork. All paperwork must be reviewed and signed by an MD.

ELIGIBILITY

The applicable NAICS code for this requirement is (621999) with a Small Business Size Standard of $18M. The Product Service Code (PSC) is Q403.

SUBMISSION DETAILS

Interested businesses should submit a brief capabilities statement package (no more than ten 8.5 X 11 inch pages, font no smaller than 10 point) demonstrating ability to provide the products/systems listed in this Description. Documentation should be in bullet format. No phone or email solicitations with regards to the status of the RFP will be accepted prior to its release.
