(Contractor must obtain and submit all permits prior to performing any work). Work under this Contract includes furnishing all supervision, labor, materials, tools, and equipment and performing all operations required for the demolition of the MLK Parking Garage. The scope includes, but is not limited to, the removal of footers, preparation of materials for disposal, loading, and transportation of materials for recycling/disposal, disposal costs, and obtaining all required permits. Additionally, the scope includes partial demolition of the south canopy over the roadway, as close as possible to the parking garage, and partial demolition of the west canopy to the east side of the roadway as close as possible to the parking garage. The garage is a three-story building and approximately 265,000 SF. Furthermore, the work includes demolition notification as required by National Emission Standards for Hazardous Air Pollutants (NESHAPS), dust control, removal & disposal of electrical work, inclusive of bulbs/ballast, removal & disposal of all mechanical systems, inclusive of elevator hydraulic oil, shafts, and tanks. All utilities, inclusive of water and sewer, must be removed and properly capped. The existing generator to be removed and disposed of. The contractor is to provide 24/7 security guard services during the entire demolition time, Maintenance of Traffic, and Stormwater Pollution Prevention Plan (SWPPP), including truck wash. The contractor must furnish & install a perimeter fence with two (2) gates, a windscreen, and a silt fence while demolition occurs. The fence shall be a permanent, to remain in place after construction, commercial-grade fence with 6-gauge wire with bottom and top rails. The fence post shall be scheduled 40. At the end of the demolition, the contractor must leave the fence to secure the perimeter, silk barrier should be removed after the demolition has been completed. The fence must be free of damage, and the two (2) gates should be functional and equipped with a locking mechanism for security purposes. Work also includes tree protection and tree removal. Technical Certification:
LICENSE REQUIREMENTS: Required Contractor's Certification: At the time of Bid and pursuant to the requirements of Section 10-3 of the Code of Miami- Dade County, Florida, the Bidder must hold a valid, current, and active: 1. Certificate of Competency from the County’s Construction Trades Qualifying Board as a General Engineering Contractor; or, 2. The State of Florida General Contractor License, as required by the Florida Building Code, pursuant to the provisions of Section 489.115 of the Florida Statute. EXPERIENCE: The Bidder must demonstrate that it has full-time personnel with the necessary experience performing similar scope of work, and to demonstrate at least ten (10) years of experience in large-scale demolition projects. Provide detailed description of at least five (5) projects similar in detail to the Project’s Scope of Work described in this Solicitation Document and in which the bidder is currently engaged or has completed within the past ten (10) years. List and describe the aforementioned projects and state whether the work was performed for the County, other government clients, or similar size private entities. The description should identify for each project (1) The client, (2) Description of work, (3) Total dollar value of the contract, (4) Contract duration, (5) Customer contact person and phone number for reference, (6) Statement or notation of whether Bidder is/was the prime contractor or subcontractor, and (7) The results of the project; provide letters of certification of final acceptance or similar project closure documentation and/or Contractor’s evaluation/performance INDEMNIFICATION AND INSURANCE REQUIREMENTS: The Contractor shall furnish to Department of Transportation and Public Works, 111 NW 1 Street, Miami Florida 33128, Certificate(s) of Insurance which indicate that insurance coverage has been obtained which meets the requirements as outlined below: A. Worker’s Compensation Insurance as required by Florida Statute 440 B. Commercial General Liability Insurance for a minimum $1,000,000 per occurrence, $2,000,000 in the aggregate. Products/completed operations. Miami-Dade County must be included as an additional for ongoing and completed operations. C. Automobile Liability Insurance covering all owned, non-owned and hired vehicles in an amount not less than $1,000,000 combined single limit. D. Umbrella or Excess liability providing excess coverage over underlying(s) B and C for a minimum $3,000,000 per occurrence/aggregate. Terms to be not more restrictive than underlying coverages. E. Pollution liability, $1,000,000 each occurrence $2,000,000 aggregate For any work within 50 feet of any railroad property Contractual Liability-Railroads CG 24 17 10 01 and Railroad Protective Liability CG 0035 for $5,000,000/$10,000,000 are required.
